how many places still use the ACF for picc placement??

I have been looking at various literature and audit sources and discover some places are still using the ACF despite not being the optimum place for picc insertion. Macmillan literature also advocates using the ACF in leaflets to patients.
Why are these places still using this route when it is clearly inferior for the patient?? Thoughts and ideas please!!

In my experience ACF placement is rare. Ultrasound assisted PICC placement has virtually removed the need for ACF placement. As for patient information I guess that the printed literature has not caught up to the digital changes. The following link shows that MacMillan now suggest the PICC will be inserted just above the elbow http://www.macmillan.org.uk/information-and-support/treating/chemotherapy/being-treated-with-chemotherapy/picc-lines.html
